Low phosphorus/low sodium recipes??

I have a friend, a vet actually, who's eyes I just opened about dog food. (Thanks BW and dogfoodanalysis!!) She admits she doesn't know much about nutrition and enlightened me as to why vets recommend crappy foods.
Anyway, she has 2 older dogs that have kidney disease, one entering the advanced stages, and she is feeding Hill's Prescription Diet KD because of the low sodium and phosphorus levels it contains. She would like to try home cooking for her dogs instead. So, does anyone have any good recipes with very low phosphorus and low sodium?

There is a kidney recipe in Dr. Pitcairn's Natural Health For Dogs & Cats, and probably more diet and homecooking information for dogs suffering from kidney disease on the internet. Kudos to you for helping a vet learn more about nutrition!!
