Sorry but I have to agree 100% with ehayes here. You are saying that you already know about the health testing that should be done on breeding dogs... but you're choosing to ignore that because you want a puppy
now. God forbid that puppy does end up with a genetic condition, I assure you, when that happens you will NOT feel so "prepared" for such a financial and emotional burden. Please keep in mind that the 2 very common health issues in the breed are heart conditions which can cause sudden death, even in very young dogs.
As for the contract, I really don't see any point in writing up one of your own, because if it isn't in the breeder's contract already then you're out of luck.
Please reconsider buying from this breeder and instead find a puppy from a reputable breeder. Purchasing a puppy should not be such a hasty decision. Not only do you risk dealing with numerous health problems that will
at the very least cost you a lot of money, but you will also be supporting unethical breeding practices by purchasing from a breeder who does not perform health testing.
